___ Health Department review fees ($50 for site plans, $75 for subdivision plats)

___ Appropriate number of copies (28 for Planning Commission, 7 for sketch plans, 7 for Forest Conservation, 12 for ZBA, 2 for HPC administrative approval, 13 for HPC applications)

___ Notarized Agent Authorization form (if applicable)

Final Plats:

___ Title Report

___ Archeological Identification/Evaluation Report OR Certification of No Archeological Resources (see Section 910)

___ If subdivision of property is 40,000 s.f. or more, the approval letter for the Forest Stand Delineation

___ Approval letter for the forest conservation plan (if associated with a preliminary subdivision, and a forest conservation plan was submitted)

___ Final HOA Documents (if an HOA is proposed)

Preliminary Subdivisions:

___ Draft HOA Documents (if an HOA is proposed)

___ If subdivision of property is 40,000 s.f. or more, the approval letter for the Forest Stand Delineation (unless a combined FSD/FCP is submitted concurrently)
