Improvement Plan Review Application

• All plans shall be prepared in accordance with the Improvement Plan Review Checklist

  New submittals shall include five copies of the improvement plans, an Improvement Plan Review Application, an Improvement Plan Review Checklist, one set of storm water management computations and a fee.

Engineering Checklist

• This form must be submitted with improvement plan review applications. This checklist offers minimum design criteria.

Standard Unit Costs Form

• Form which provides unit costs which must be utilized to establish a bond amount for grading and sediment and erosion control and public improvements within the public right-of-way. Please note that the forest conservation wetland and non-wetland areas must be bonded with the Forest Conservation Agreement not included in the Public Works Agreement and Bond.

Grading/Public Improvements Permit

• Form to be completed for submission of a Grading/Public Improvements Permit. A Construction Inspection Fee (based on amount to be bonded) must be paid at time of application for a Grading/Public Improvements Permit.

Public Works Agreement

• Agreement executed between a developer and the City which guarantees completion of public improvements and sediment and erosion control measures in excess of 5,000 square feet of disturbed area. Contact the Engineering Department to obtain this form.

Policy on Acceptance of Security

• Policy which specifies acceptable sureties which may be submitted as guarantees for public improvements with public works agreements.

Performance Bond

• Performance Bonds submitted with a Public Works Agreement must be completed on this form.

Sample Letter of Credit

• Sample Letter of Credit format which may be used as a reference for required information.

Traffic Request Form

• Form used to submit a request to the traffic engineering division.

Fee Schedule

• General fee schedule for engineering department.

Inspection and Maintenance Agreement of Private SWM Facilities and Deed of Easement

• Forms for private stormwater management facilities (ponds, outfall structures, etc. to be maintained by the developer). Contact the Engineering Department to obtain these forms.

SWM Waiver Request

• This form is used to request a waiver of stormwater management quality and/or quantity.
