Programs and Services
  • Founded in 1968, the Frederick Community Action Agency operates a wide-range of programs and services to assist families and individuals that are lower-income or homeless.  The Frederick Community Action Agency is sponsored by the City of Frederick and the Friends for Neighborhood Progress, Inc. with support from the Frederick County Government and the United Way of Frederick County.

Primary Health Care Clinic and Health Care Programs

  • The Frederick Community Action Agency operates a primary care clinic that is staffed by licensed nurse practitioners, registered nurses, case managers, medical assistants, and physicians.  The clinic is open Monday through Friday, please call 301-600-1506 to make an appointment.

Primary Health Care Clinic and Health Care Programs
  • Health Care for the Homeless: The Health Care for the Homeless (HCH) program provides primary health care, addictions treatment, and supportive services to people who are homeless.  The FCAA operates a primary health care clinic that is staffed by nurse practitioners, physicians, nurses, and case managers.
  • Eligibility criteria:  Homeless (e.g., living on the streets, in cars, in shelters, doubled-up, or in other locations not meant for human habitation).
  • Primary Adult Care Program: The state-funded Primary Adult Care (PAC) Program provides primary health care, pharmacy assistance, case management, and ancillary services to people eligible for enrollment in the program.
  • Eligibility criteria:  Must complete an application at the FCAA, provide documentation of income, and meet income eligibility criteria.  Once enrolled in the PAC Program, participants must be enrolled with Maryland Physicians Care MCO or Priority Partners MCO and select the FCAA as the health care provider.

Primary Health Care Clinic and Health Care Programs
  • Access to Care Program:  Funded in part by the Maryland Community Health Resources Commission, the Access to Care (ATC) Program provides primary health care services for adults and children who are uninsured and have total household incomes at or below 200% of the federal poverty level (FPL), as adjusted for the size of the family; for example, an annual income of $40,000 or less for a family of four.  Eligibility criteria:  Apply at the FCAA and provide documentation that the total household income is at or below 200% of the federal poverty level, as adjusted for the size of the family.


  • Medical Assistance Provider: The FCAA is a participating provider in the Maryland Medical Assistance (Medicaid) Program and is currently accepting both adult and pediatric patients that have active Medical Assistance and have enrolled with Priority Partners MCO.  The FCAA also accepts patients with Unrestricted Medical Assistance.  Eligibility criteria:  Must have Medical Assistance, be enrolled with Priority Partners MCO or Maryland Physicians Care MCO, and select the FCAA as the health care provider or have Medical Assistance – Unrestricted.

  • Medbank Program: Through the Medbank Program, FCAA case managers help people to access free prescription medications made available through patient assistance programs operated by pharmaceutical manufacturing companies.
  • Eligibility criteria:  Must complete an application at the FCAA and meet income eligibility guidelines (the income eligibility guidelines vary for each different pharmaceutical manufacturing company).

Case Management Services and Homeless Assistance Programs
  • The Frederick Community Action Agency provides a wide-range of case management services to assist families and individuals that are homeless or lower-income.


  • Case Management can be defined as individualized assistance and overall management and development of goals and strategies to address individual problems relating to the client household.

Case Management Services and Homeless Assistance Programs
  • Service-Linked Housing Program: The Service-Linked Housing (SLH) Program provides case management and outreach services to lower-income renters and homeowners residing in permanent housing.
  • Eligibility criteria:  Lower-income families and adults who are in need of assistance and would benefit from case management services.


  • PATH Project: The PATH Project provides case management, outreach, transportation, and addictions treatment services to people who are homeless and have a serious mental illness.  Initiated in 1992, PATH Project staff work closely with therapists and psychiatrists from other agencies to ensure adequate access to mental health treatment services for homeless people.  Eligibility criteria:  Seriously mentally ill and homeless (e.g., living on the streets, in cars, in shelters, or in other locations not meant for human habitation).

Case Management Services and Homeless Assistance Programs

  • Transitional Shelter: The Frederick Transitional Shelter for Homeless Families is a 31-bed shelter that was built by the City of Frederick and is operated by the FCAA.  The shelter provides transitional housing and a wide range of supportive services for homeless families and physically disabled adults.  The Transitional Shelter serves approximately 60 households on annual basis.
  • Eligibility criteria:  The transitional shelter serves homeless families (i.e., married couples, two-parent families with children or single-parent families with children) and medically or physically-disabled adults.

Housing Counseling Services and Housing Assistance Programs
  • HUD-Approved Housing Counseling Agency: The FCAA is a HUD-Approved Housing Counseling Agency and provides pre-purchase, default, reverse mortgage, and rental housing counseling for renters, homeowners and first-time homebuyers.
  • Eligibility criteria:  Families and adults in need of housing counseling services such as mortgage default counseling, reverse mortgage counseling, or first-time homebuyer education.


  • Homebuyer Education Classes:  The FCAA offers Homebuyer Education Classes on a regular basis to assist first-time homebuyers that are working to purchase their own home or to better understand the process of obtaining a mortgage.  Families that graduate from the Homebuyer Education Program often benefit from follow-up counseling on an individual basis with an FCAA Housing Counselor.
  • Eligibility criteria:  Families and adults in need of housing counseling for first-time homebuyers.

Housing Counseling Services and Housing Assistance Programs
  • Weatherization Assistance Program: The Weatherization Assistance Program provides home weatherization services for income-eligible households living throughout Frederick County.  The family may rent or own their home, but renters must obtain landlord permission.
  • Eligibility criteria:  Must have incomes at or below 175% of the federal poverty level; families that are eligible for the Maryland Energy Assistance Program (MEAP) operated by the Frederick County Department of Social Services are automatically eligible for weatherization services.


  • Housing Rehabilitation Program: Utilizing funds from a variety of resources, the FCAA provides small grants (usually less than $10,000) to lower-income homeowners to address emergency and/or health and safety repairs in owner-occupied housing.
  • Eligibility criteria:  Lower-income homeowners that are in need of housing rehabilitation assistance to address emergency and/or health and safety repairs.

Food and Nutrition Programs
  • Frederick Soup Kitchen: The Frederick Soup Kitchen provides an evening meal seven days a week and serves over 2,000 meals per month.  Eligibility criteria: None, free meals are provided to those in need.


  • Summer Food Service Program: The program provides a free lunchtime meal to children age 18 and younger and targets low-income children who receive free or reduced-price lunches during the school year.  The nutritious and balanced meals are served Monday through Friday during the months of June, July, and August.
  • Eligibility criteria: Free lunchtime meals are provided to children and their parents in specific parks or neighborhoods (sites must qualify under USDA regulations); contact the FCAA for a list of locations where meals are served or to inquire about establishing a new site.

Food and Nutrition Programs
  • Frederick Foodbank: The Frederick Foodbank provides a three- to five-day supply of food to families and individuals that are experiencing an economic crisis.
  • Eligibility criteria: None, free food items are provided to families in need of food, participants must complete an intake form.

Donations and Volunteer Opportunities
  • Donations
  • The FCAA accepts donations of non-perishable foods, personal hygiene supplies, diapers, underwear, socks, gloves, and winter hats.  Monetary donations are also accepted.  For more information contact Sarah McAleavy, Coordinator at 301-600-6263 or by email at smcaleavy@cityoffrederick.com


  • Volunteers
  • Volunteers are needed to help stock and distribute food through the Foodbank Program and to serve prepared food in the Soup Kitchen.  Professional volunteers (physicians, nurses) are also needed for the health care clinic.
