Sec. 23-15. Application.


(a) Application for a new or renewal taxicab driver’s license shall be filed with the taxicab administrative agency on an application form provided by the agency.
(b) Applications filed pursuant to subsection (a) shall be sworn before a notary public and contain the following information about the applicant:
(1) Full name;
(2) Current business address, telephone number and e-mail address;
(3) Current home address, telephone number and e-mail address;
(4) Places of residence for the preceding two (2) years, including, length of time at each residence;
(5) Date of birth;
(6) Country of citizenship and if the applicant is a permanent resident of the United States;
(7) Employment history for the preceding two (2) years;
(8) List of any crime, or serious motor vehicle violation;
(9) A copy of the applicant’s driver’s license; and
(10) A list of any other names and/or aliases used.
(c) Applicants must also supply the taxicab administrative agency with a certified copy of the applicant’s complete driving history from the department of motor vehicles, by whatever name it is known, for any state or the District of Columbia which the applicant has ever been licensed.
