(a) All original applications for permits shall be made upon forms provided by the taxicab administrative agency and shall be prepared and signed by the applicant.
(b) At the time of making such application, the applicant will be given a printed copy of this chapter, unless that person has previously received a copy, and they shall certify in their application that they have received and read the provisions of this chapter and agree to comply with the requirements thereof.
(c) It shall be necessary for any person or entity seeking to obtain a possessory interest in a permit to fill out an application.
(d) If the applicant is a corporation, the application shall provide the name and address of the president and secretary. If the applicant is a partnership, the name and address of each partner and the extent of the interest that each has in the business shall be listed on the application. (Ord. No. G-04-10, 6-3-04)
